BRIT KAVOD (HONOR COVENANT) AND RELEASE FORM
A brit ("breet") is a covenant. We ask Shoresh parents to read through this section and enter into a brit, a covenant, with their child(ren), with other families, and with our Shoresh community to bring kavod (respect) and shalom (peace) to Shoresh each week.
AS A LEARNER, I agree that I will:
Remember that Shoresh and KAM Isaiah Israel is a place of worship and learning.
Be respectful of our building, our online learning environment, and everything in and around it. Therefore I will not litter, deface, or damage anything, nor will I disrespect online learning platforms.
Be respectful of everyone in our community:
I will respect my peers by paying attention when others are speaking, using kind language and manners, keeping my hands and body to myself, not using anything that does not belong to me, not putting down others, and listening to others’ opinions.
I will respect my teachers by following the class expectations, listening while the teacher is talking, and doing what is asked of me at Shoresh.
Be respectful of myself by always being safety conscious. I will not run, climb, play wildly, or throw anything (except as appropriate in designated areas). I will use online platforms as they are intended and as my teacher directs me.
I will not treat others in ways in which I do not want to be treated.
AS PARENT(S), I/we will:
Share responsibility for our child as a learner who is learning how to become a part of the Jewish people, by approaching our child’s exploration of our heritage and trying to make meaning of it as a family endeavor and responsibility.
Share responsibility for the other learners and the teachers sharing in our learning experience by helping our child to be prepared for class, arriving on time, participating in Shoresh activities, and reinforcing Jewish values at home. We will share with Shoresh the responsibility of monitoring our child’s progress.
AS A SCHOOL, we will:
Strive to treat every child with respect, compassion, and active interest
Do our best to teach Jewish values, tradition, and knowledge
Incorporate all learning styles and abilities whenever possible
Reinforce the terms laid out in the inclusion policy
We have read the Brit Kavod/honor covenant and discussed it with our family. We understand that by attending Shoresh we are agreeing to the terms of the Brit Kavod/honor covenant.

Release form
In the event that we/I cannot be reached in an emergency, I/we authorize a KAM Isaiah Israel representative to seek medical attention for my children.
I give permission for photos and videos of my children to be used for publicity purposes, including but not limited to KAM Isaiah Israel emails, website, and social media.
I give permission for online learning sessions to be recorded for the purposes of observation and supervision for Shoresh teachers. Videos and recordings will be used exclusively by KAM Isaiah Israel and Shoresh staff and teachers to review and will not be distributed beyond the faculty.
The above children has/have permission to participate in all class field trips during the 2020 - 2021 school year. I will be notified in advance of all trips. I will notify the school in writing if my children are unable to participate.
I release KAM Isaiah Israel Congregation, its trustees, officers, employees, agents, and all other persons participating in our chaperoning such field trip from all claims which I or my children may have for personal injuries (or emergency medical care related to such injuries), known or unknown, which my children may incur by participating in such activity.

TUITION INFORMATION
Jewish education is an investment in the Jewish future. If you find that these fees present your family with a financial hardship, please contact Lauren Reeves, Religious School Director, at 773-924-1234 to discuss special payment arrangements or scholarship opportunities. Financial concerns will NOT prevent your child from receiving a Jewish education.
Shoresh is supported by KAM Isaiah Israel members, and we encourage all Shoresh families to become members of the congregation. Tuition for non-members is 1.5 times the member tuition rates. To join KAM Isaiah Israel, contact the KAM Isaiah Israel office. Members must be in good standing for 2019-20 and renew membership for 2020-21 to receive the member tuition rate for the 2020-21 school year.

The non-member fee is 1.5 x regular tuition for families who are not members of KAM Isaiah Israel. If you are not a formal member of our congregation, KAM Isaiah Israel will adjust your account and send you a statement via email.
Tax-deductible donation
If you would like to make an optional tax-deductible donation to Shoresh learners in need of financial assistance or a donation towards special Shoresh programs, please enter the amount above.
Tuition will not be refunded after the fourth week of Shoresh. The exception is for 8th-10th graders. KAM Isaiah Israel will refund the portion of tuition that pays for overnight trips if the trips do not take place during this school year.
Fees:
The b'nei mitzvah fee should be paid for the school year during which the learner's b'nei mitzvah takes place.
A Hebrew fee applies to learners beginning Shoresh in 4th grade or later.
Families who are enhanced dues members with tuition included in their membership dues will need to pay these fees as applicable.
Families with three or more children in Shoresh can pay half price tuition for one child at the lowest tuition level, excluding retreat fees.
